Keliane has more than 20 years of nonprofit leadership experience in planning, implementing, and evaluating community health programs. In prior roles, she led business development, fundraising, and communications functions for health-related organizations.
With Pear Associates, Keliane uses her skills to boost the impact of small to mid-size nonprofits striving to improve the health of the communities where we work, live, and play. This includes providing project oversight and advisement for Mass General Hospital’s Care for America’s Aging, a four-year demonstration project to create a new home care model that elevates and supports the role of home health aides. She skillfully facilitates strategic planning, organizational assessments, and business development with clients, such as Health Resources in Action, The National Center to Reframe Aging, and SHARE Outreach.
Keliane has an enthusiasm for community engagement and the power of collective impact. A champion for healthy aging, Keliane was trained by the Reframing Aging Initiative. During 15 years with Granite VNA, she facilitated community needs assessments that led to innovative and sustainable programming for fall reduction, chronic disease self-management skills, advanced care directives, caregiving education, and support groups and caregiver resources. In partnership with public housing and the local healthcare system, Keliane launched a community health worker program to improve people’s confidence in managing their chronic health conditions while lowering emergency department visits.
Keliane earned a Master of Education and a Bachelor of Science in Health Education from Plymouth State University and is a Master Certified Health Education Specialist. She lives in New Hampshire with her husband, daughter, son, and sweet chocolate lab. Keliane loves skiing, the ocean, and cheering on her kids, the Celtics, and Patriots.
